b) I don't think Lucifer per se was ever 9th level. Asmodeus in contemporary D&D terms (http://www.enworld.org/showthread.php?t=24461) is 60th level and a god, making him (among other things) unkillable except by another god of higher rank. In LOTRO terms, that would make him 150th level at the very least.
In comparison, a 40th level mosquito ain't so tough.
I'd have to dig out my older D&D books to get stats from previous editions, but even in the heady days of 1979 one didn't just whomp on the king of hell as if he were just another monster.
